Support Plan Template

Following a number of enquiries from Sheltered / Supported Housing providers, who seemed to be struggling with the concept of Support Plans and commented that existing models seem very lengthy, The Sheltered Housing Network have worked on developing a model support plan template. 
 
The Support Plan has been designed with the aim of providing an easy to follow format that is not too time consuming, yet provides relevant information that complies with the philosophy and ideals of Supporting People.  With this aim in mind, the original design was constructed by a working party of sheltered & supported housing front line staff.  It was then shared with a number of service managers in Local Authorities & RSL's for their comments.  This led to some further adjustments being made and the overall feedback has been very positive.
 
In line with the amended Quality Assessment Framework SHN decided to revise this support plan template and again sought feedback from Supporting People teams.  The revised SHN Support Plan Model Template was sent to regional supporting people teams across the UK asking them for their comments.  The overall feedback was very positive. Many identified the template as being clear, concise, effective for its purpose, easy to use and that it met the core requirements of supporting people. 

With the aim of assisting organisations in designing effective support plan documentation, The SHN model support plan template is designed to:

bullet Provide an effective tool to enable organisations to produce documentation that is customised to meet their specific requirements (this can be done 'in-house' by the organisation or by SHN, whichever best suits you)
bullet Be easy to use and understand, so as to assist staff to adopt a Pro-Active approach towards conducting and reviewing support plans
bullet Be effective in identifying the varying support needs of users
bullet Constructively plan effective service delivery to match the users support needs  
bullet Enable organisations to identify trends in dependency levels
bullet Give clear evidence of the provision of a needs led service that actively consults with its users

The Support Plan Template and Review document is available FREE to all SHN corporate members and to other registered sheltered housing providers at a charge of £25 to cover administration costs.
