Pro-Active Support Planning - Identifying and Monitoring Outcomes

Who Course is for:

Staff working in and delivering support services to tenants/lessees living in sheltered housing and who, through their role: • Provide support to service users, • Liaise with other agencies to obtain appropriate support for service users

 

This course may also be beneficial for those line managers with a responsibility for staff involved in the running of sheltered housing schemes. It will enhance understanding and learning about the practical issues staff may face and enable managers to provide a more positive role in supporting such staff.

 

Course Content

  • The requirements of Supporting People in relation to Support Plan Outcomes

  • Providing effective support that will enhance the users self esteem and independence

  • Identifying the outcomes of the provision of support that sheltered housing users require

  • Effective content in support plan documentation to monitor the outcomes of support provision

Overall aim of the course

  • To enable staff to understand the support plan process in relation to identifying and monitoring outcomes.

  • o develop a pro-active approach that is ‘person centred’ when setting up and reviewing support plans.

Course Objectives - What you should be able to do by the end of the course

  • List the benefits of identifying and monitoring outcomes when using structured support plans

  • Define the requirements of Supporting People in relation to Support Plan Outcomes

  • Identify what is useful information that could be included in support plan documentation

  • Explain appropriate action to provide and/or obtain support within the constraints of the role of sheltered housing staff

  • Address key issues around identifying and monitoring outcomes when planning support

  • Demonstrate effective methods that can be used to help establish the support needs of service users

Summary

The course is designed to be flexible and the content and setting can be varied to meet the particular requirements of individual groups.

 

Participants will be encouraged to adopt a supportive and positive approach to the group as a whole. It is hoped that this will allow individuals to achieve greater proficiency and confidence through taking part in a stimulating and enjoyable learning programme.
